                                Chief Executive Officer

                                Jesús Hernandez

                                Jesus brings to Community Choice extensive expertise and experience in start up and implementation of programs including outreach and education experience with disadvantaged populations. Jesus completed a Bachelor degree from Washington State University in Business Administration and a Masters in Public Administration from University of Washington. He has diverse experience in administration of programs in workforce development for people with disabilities and minorities, small business development, youth leadership programs, strategic planning, policy development, public education and workforce investment.

                                Leadership: Jesus has served over ten years as an elected School Board Member of the Wenatchee School District and was appointed by both Governor Locke and Governor Christine Gregoire to the state's Higher Education Coordinating Board. Jesus has been actively involved in various civic activities and non-profit boards in his community.

                                Expertise: Leadership development; grant writing, grants administration and program implementation; outreach and community development; program design; group facilitation; group training in cultural competency in the workforce; health information systems architecture and implementation; marketing; bilingual, bi-literate, bicultural in Spanish/English.


                                Chief Operations Officer

                                Allen Dietz

                                Allen Dietz has extensive experience in non-profit management and local and state government service. Allen served as the Executive Director of the North Central Washington Rural Health Foundation in Wenatchee, Washington. For the past eight years he has also been a consultant with Project STAR (JBS International) on performance measurement and evaluation issues for national service programs, is a national VISTA trainer, and has extensive experience working with AmeriCorps, VISTA, and Senior Corps programs throughout the country. Allen was Director of Training, Planning, and Evaluation with the Texas Commission on Volunteerism for three years, and directed a large statewide AmeriCorps program in Texas for four years.

                                Allen has volunteered as a national disaster mental health specialist and trainer with the American Red Cross, and has responded to floods, tornadoes, and tropical storms in Texas, the World Trade Center terrorist attack and the crash of American Airlines flight 587 in New York City, and to wildfires in California and Washington.

                                Allen graduated with a BA from the State University of New York at Stony Brook and did extensive graduate work at the University of Texas at Austin. He has a broad background in fundraising and proposal writing for non-profit organizations, and has been actively involved in training health and human service professionals for more than 25 years.


                                SHIBA Coordinator

                                Maria Ontiveros

                                As Director of Programs, Maria provides general operational leadership and is responsible for coordinating and reporting on grants and programs. She also puts together the bi-monthly Newsletter and her superb people skills support the ongoing communications with our members.

                                Maria is also the the lead coordinator of the SHIBA HelpLine program serving Chelan, Douglas, Okanogan, Grant and Adams counties. Maria is our resident expert on eligibility and benefit matters related to the wide array of healthcare benefits. This includes Medicare, Medicaid, Basic Health and other local, state and federal public and private health insurance programs.

                                Maria is a proud graduate of Wenatchee Valley College and is currently pursuing a Bachelors degree. She has over eight years of experience in the healthcare field and is bilingual bi-cultural in Spanish/English.



                                Office Manager

                                I like to think of myself and the work that I contribute in the role of Office Manager as “the hub of the wheel”.  It’s not very glamorous, but extremely vital.

                                After graduating from Iowa State University with a Bachelors of Science degree in Textiles, I spent five years working in London, England as a buyer for a well-known department store. I definitely broadened my list of life experiences with many trips throughout Europe as part of my work. My next stop was in Johannesburg, South Africa for two more fascinating years of social and cultural interaction with other nationalities.

                                When I returned to the United States, I chose to pursue my love of interior design and worked as a designer for the next 25 years in Washington State. As I worked with my clients, I developed a desire to serve in a different way and to a specific group: seniors.

                                For six years prior to joining the team at Community Choice, I served as the Office Manager of a large retirement and assisted living facility. I found this was a place where my people skills combined with my finely honed organizational skills could be utilized well.

                                I am pleased to continue to work in an environment where I can contribute to the betterment of the lives of those we serve.


                                Database Specialist

                                Terry is currently employed by Community Choice to provide specialized Technical Assistance (TA) to WIREC clients deploying their respective Electronic Health Record Systems seeking to reach “meaningful use” criteria as dictated by the Office of the National Coordinator (ONC) for Health Information Technology.

                                Roosevelt Hubbard

                                Roosevelt Hubbard is working on our Federally funded WIREC project as a System Administrator in the Seattle area. Roosevelt has extensive experience as a system administrator and technical operations manager in a variety of IT settings at Microsoft and at other organizations in the Seattle area.



                                IT Manager

                                Mike Ellis

                                Mike has been working with IT since the late 90's. His certifications include the industry standard CompTIA A+, Network+, Server+, and iNet+, as well as the CIW and Microsoft Certified Professional certs. His education credentials include an associates degree in computer administration and a second associates degree in computer science.

                                Mike has set up multiple computer labs for non-profit organizations, ran a computer refurbishment center, and put together this website :)
