Library cardholders, over the age of 18, can check out a blood pressure monitor kit for up to two weeks to self- monitor their blood pressure.

The Chronic Disease Self-Management Program supports people with different or multiple chronic health problems. Program is led by trained and certified peers with first-hand experience. We discuss techniques on how to deal with frustration, isolation, appropriate exercise, fatigue, pain, medication management, emotion management, and much more.

Chronic Pain Self-Management Program

The Chronic Pain Self-Management Program was developed for people who have a primary or secondary diagnosis of chronic pain. Pain is defined as being chronic or long term when it lasts for longer than 3 to 6 months, or beyond the normal healing time of an injury. Program is led by trained and certified peers with first-hand experience. We discuss dealing with frustration, fatigue, isolation,  poor sleep, appropriate exercise, appropriate use of  medications, and much more.

Diabetes Self-Management Program

The Diabetes Self-Management Program supports people with all types of diabetes. Program is led by trained and certified peers with first-hand experience. We discuss techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and all of the emotional problems associated with this disease. Our material has been reviewed by physicians, diabetes educators, dietitians, and other health professionals.
