Community Choice Healthcare Network physician members are committed to providing excellent health care to their patients. Our ability to improve patient services is greatly enhanced by the feedback that our patients & consumers provide us.  This site allows you to access and complete surveys with a Survey Code that our participating members have issued to receive feedback.

To maintain the integrity of the data as well as patient  anonymity, each patient/client is provided this Survey Code during a doctor's visit to complete their respective survey.  Community Choice will maintain your anonymity and provide your physician with your valuable input.
