The initiation of the Community Choice Health Record Bank was funded by the Washington State Health Authority to support a patient-centric system that allows healthcare consumers to have access to important health information under their total control.  When you subscribe to the Health Record Bank: 

1. Your identity is authenticated by one of our certified Registrars and a secure Personal Health Record is created on your behalf. 

2. You select from a list of health care providers all who will automatically send your health data to your Personal Health Record. 

3. You will receive guidance on how to effectively manage your Personal Health Record. You may assign qualified access to trusted members of your family and/or healthcare providers.

4. On a pre-determined schedule, your most current data will be uploaded to your Personal Health Record.  It is important that you regularly verify that the health information uploaded to your Personal Health Record is correct.  For example:  the prescriptions generated into your Personal Health Record may not be what you actually purchased or are choosing to take. 

Only you can verify and insure the accuracy of your Personal Health Record.

  • Wenatchee Valley Medical Center and member clinics (completed)
  • Lake Chelan Clinic
  • Central Washington Physician Services (completed)
  • Brewster Medical Center
  • North Valley Hospital
  • Quincy Valley Medical Center
  • Mid Valley Hospital
  • Washington State Immunizations Registry - Child Profile

    Other organizations will follow as funding allows.  Our goal is to get all our Community Choice healthcare members and other providers interfaced to our Health Record Bank.
