Community Choice Basic Membership
(Membership is dependent on board approval)

Membership Services Include:
  • Regional HC Promotion and Medical Staff Retention Services
  • Spanish Radio Outreach and Education
  • Regional Access
  • Patient Navigation Service Training
  • Coordinating Site Volunteers

Monthly Fee Structure:
  • Small Clinic: $270/month
  • Physician: $90/month
  • Mid-level: $50/month
  • Small Hospital: $1,000/month
  • Large Hospital: $2,500/month

+ An Annual Fee of $400
Community Choice Grant Services

Services Include:
  • Searching for Grants
  • Grant Application
  • Grant Administration

Monthly Fee Structure:
  • Physician: $25/month
  • Clinic: $50/month
  • Hospital: $50/month

+5% of total funds secured up to 5K
