Living Well with Chronic Conditions, is an evidence-based Chronic Disease Self Management Program developed by Stanford University. The program is a FREE six-session workshop to help you make the most out of life, while managing chronic conditions (such as arthritis, asthma, cancer, chronic pain, depression, diabetes, fibromyalgia, heart disease, migraines, obesity, and osteoporosis). Each workshop consists of 2 1/2 hour weekly sessions where you will learn skills and be given tools to:
